VIVE 2023 was a great success for Together Senior Health. Conversations at the conference made clear that as our population ages care at home is increasingly critically important. It is not only safer for this population but there is a correlation to better outcomes. Digital Health is that transformative enabler to “meet members where they are” and deliver the services they need to age in place better.
1 in 9 people in this population are living with Alzheimer’s or dementia. While this may not be their primary diagnosis, the clinical and financial impact on other morbidities is only exacerbated by this disease. In fact, members living with dementia cost 3x that of their peers in the same health plan.
